The aim of this study is interesting. Have the statistics and the calculation been reviewed by an experienced specialist in statistics? Presuming there are no problems in these areas, then the statistically significant differences in the study are interesting in research. However, it’s not practical to apply these results clinically in differentiating psoriasis from psoriasiform lesion.

ROLE OF LEUKOTRIENE RECEPTOR ANTAGONIST MONTELUKAST IN THE TREATMENT OF CHRONIC URTICARIA: A HOSPITAL BASED STUDY

The paper entitled “ROLE OF LEUKOTRIENE RECEPTOR ANTAGONIST MONTELUKAST IN THE TREATMENT OF CHRONIC URTICARIA: A HOSPITAL BASED STUDY” deals with a interesting alternative therapy for chronic urticaria. Chronic urticaria is a challenge in front of the clinician. The results demonstrated that the leukotriene antagonist has certain place in the treatment of the disease. Although the urticaria patients were not stratified regarding the type of chronic urticaria according to the present international guidelines, the study offers a proof that montelukast can be effective in chronic urticaria.

This is very interesting article regarding a challenging condition. The chronic urticaria is a disabling disease, with a diffficult management in some cases. This work evaluates the response a new promising therapy for this contidion. Authors present a study on 35 patients with chronic urticaria to evaluate the response the montelukast therapy. While it is non-placebo controlled trial, the sample has a considerable size to obtain preliminary conclusions.
The features associated with patients included in the study are well-described, which gives the chance to make some hipothesis such as the one made by the authors stating that NSAID induce urticaria may have a good response to montelukast therapy.
The main limitation in the value of this manuscript I have found is regarding the method used to evaluate the therapy with montelukast: this study is a single-blinded, non-placebo controlled trial with no statistical analysis, which makes the conclusions more probabilistic rather than causal. Furthermore, no scores to evaluate the response to montelukast was used (such as Urticaria activity Score or Visual Analogue Score, which are used in other urticaria-related studies). In conclusión, the article has a good value for the disease evaluated on it, in order that chronic urticaria is a disabling and challenging dermatological condition. However, the method issues have the conclusions obtained to be considered preliminary.
Finally, I am agreed with the statement of the authors that more studies are warranted to confirm the results observed in this article.
ANTINUCLEAR ANTIBODY LEVELS IN POLYMORPHIC LIGHT ERUPTION AND THEIR RELATION TO THE SEVERITY OF DISEASE

The paper “ANTINUCLEAR ANTIBODY LEVELS IN POLYMORPHIC LIGHT ERUPTION AND THEIR RELATION TO THE SEVERITY OF DISEASE” studies the ANA status in patients with PLE and matched controls. No difference was found between both groups. The study contributes to the understanding of the autoimmune phenomena in PLE.

PUSTULAR PSORIASIS RESPONDING TO PROBIOTICS – A NEW INSIGHT

Psoriasis can be difficult to treat after each relapse of diseases, what we can do when all the treatment methods on basis of evidence are not effective. Sometimes we or patients are searching the alternative ways to reach the result. In this respect the article, where the authors described successfully treatment of resistant form of pustular psoriasis, with probiotics, is very interesting. The authors have made courageous decision to stop all traditional medication in patient with severe pustular psoriasis, which can have serious complications and fortunately they have reached excellent result.

A RARE CASE OF HERPES ZOSTER OTICUS IN AN IMMUNOCOMPETENT PATIENT

I think this case report illustrates those occurring in our clinical emergencies. The peculiarity is that it was an early stage that most often, in my experience is not common and even less in elderly immunocompetent. With life expectancy increasing we may have to make a diagnosis of new cases.
