Dermatoze lichenoide reprezintă un termen care descrie boli inflamatorii cutanate caracterizate clinic prin leziuni papuloase, iar histologic prin infiltrate inflamator limfocitar ?în bandă? în dermul superficial. Lichenul plan nu este o dermatoză eritemato-scuamoasă, dar este încadrată în acest grup datorită striurilor Wickham.
Lichenul plan este rar raportat la copii, 2-3% [1]. Lichenul plan nu a fost descris niciodată la vârsta mai mica de 5 luni [2], iar în mica copilărie este extrem de rar (dintr-un lot de studiu de 87 de cazuri doar 9 au fost raportate la vârsta sub 2 ani) [3].
Tabloul clinic este sugestiv prin prezen?a de papule violacee,? strălucitoare?, mici de 2-3 mm, poliglonale, cu o suprafa?ă plată acoperită de o re?ea fină alba, numită re?eaua sau striurile Wickham (Fig. 1a ? 1d). Uneori leziunile papuloase pot forma, prin coalescen?ă, placarde mai mari, neregulate, bine delimitate.
Pruritul este prezent, este intens ?i fără ritm circadian. Localizările preferen?iale sunt: fa?a anterioară a pumnului, regiunea lombară, fa?a anterioară a gambelor; specific la copii este dispozi?ia liniară. Fenomenul Koebner poate fi prezent, explicând de ce leziunile de lichen plan se localizează frecvent la nivelul genunchilor ?i coatelor (copilul mic se târă?te sau se joacă pe podea). Leziunile dispuse la nivelul scalpului se asociază cu alopecie. Manifestările unghiale sunt considerate absente [4] de?i, recent, au fost raportate cazuri de pterigion dorsal (fuziunea rebordului proximal cu tableta unghială) sau chiar leziuni distrofice afectând toate unghiile [1].
