Supernumerary nipple and pigmented growing lesion on the breast

A 39 years old woman with family history of melanoma was recommended from the surgery department for a pigmented lesion on the right breast. The patient observed between other known pigmented nevi this pigmented and slowly but continous growing lesion since 8 months ago. On examination, there was a supernumerary nipple on the superior part of the breast and a  nonhomogenous pigmented lesion with a brown color, a slight warty surface, that was firm and mobile with the skin and had discrete rim of pigmented globules and streaks at the periphery suggesting a nevus Spitz –like pigmented lesion on her inferior part of the breast (Fig. 1a). Dermoscopy revealed a 7,5-mm lesion with light brown fingerprint-like parallel structures at the periphery, ‘fat fingers’ (the gyri of a cerebriform central area) (Fig. 1b). A presumptive dermoscopic diagnosis of supernumerary nipple and benign seborrheic keratosis was made.
