At The London Transgender Clinic, male to female breast augmentation is part of the Mtf transition and a popular procedure. MtF transgender individuals may develop some breast tissue after hormone therapy, but the size may remain small. For larger breasts, a transgender individual needs other types of breast augmentation techniques, and natural ones are strictly recommended.

Physiological Gynecomastia is the development of prominent breast tissue in the male. The condition can develop during puberty and sometimes resolve on its own. Other causes of gynecomastia include exposure to estrogens, androgens (often taken secretly for body building), marijuana, and some medications.(spironolactone).

In many countries marijuna is legally banished or forbidden, but in these lands it is obvious that substitutes and surrogates are available as simplest food supplements that contain cannabinoids or derivatives that are not psychoactive and so no dangerous for mental health.

The Authors will discuss this argumentation below.

Gynecomastia is defined as benign proliferation of glandular breast tissue in men.

Physiologic gynecomastia is common in newborns, adolescents, and older men. It is self-limited, but can be treated to minimize emotional distress and physical discomfort.

Nonphysiologic gynecomastia may be caused by chronic conditions (e.g., cirrhosis, hypogonadism, renal insufficiency); use of medications, supplements, or illicit drugs; and, rarely, tumors. Discontinuing use of contributing medications and treating underlying disease are the mainstay of treatment. Medications, such as estrogen receptor modulators, and surgery have a role in treating gynecomastia in select patients. Treatment should be pursued early and should be directed by the patient.

Although the adult male breast contains minimal amounts of adipose and glandular tissue, there is potential for proliferation if estrogen or progesterone levels increase. Gynecomastia, which can be physiologic or nonphysiologic, occurs when the estrogen-to-testosterone ratio in men is disrupted, leading to proliferation of glandular breast tissue, and the routes are diverse: eg, exaggerating the assumption of androgens to alter the estrogen/androgen balance [1].

The Authors propose the employ of food supplements based on fibers of Agave sisaliana and cannabinoids by oral route to improve the breast in the volunteer MtF, even though a complementary treatment with natural herbs to mantain breast solid and prominent is always advisable, and other natural remedies to mantain the glamour and the prominency of the breast (ancient traditional and popular remedies) will be reviewed at the end of this modest contribution.

Agave sisalana is a plant belonging to the Agavaceae family [2,3]. Sisal juice is constituted of steroidic saponins which are precursor molecules of many pharmacologically active steroids. These precursor molecules can act in brain structures that are related to the modulation of emotional disorders.

Transgender individuals have a gender identity that differs from their assigned sex; the term includes gender nonbinary individuals who do not identify as exclusively male or female [4].

These symptomes manifest themselves as:

Here follow a digest of some food supplement containing derivatives of THC, available even in the regular or on line market all over the world.

Some of the notable alternatives include:

CBD (CANNABIDIOL)

CBD, or cannabidiol, is the most prominent cannabinoid in hemp plants and is just as equally researched as THC for its interactions with the endocannabinoid system to support relaxation, stress, sleep, and mood. Unlike THC, CBD doesn’t produce intoxicating effects, even at high doses. Some CBD products, such as full spectrum CBD, may include low doses of THC (up to 0.3%), but there are plenty of THC free CBD options as well.

CBG (CANNABIGEROL)

CBG, or cannabigerol, is sometimes referred to as the “mother cannabinoid” because this compound is a precursor to other cannabinoids, including THC and CBD. Even though it’s present in smaller quantities in mature cannabis plants compared to CBD and THC, CBG has been found to have potential wellness effects. CANNABINOIDS

DELTA-8 THC

When people talk about THC, they’re usually referring to delta-9 THC, which is the most abundant and well-researched form, and delta-8 THC is a minor cannabinoid isomer. The only difference is its molecular structure and concentration in cannabis plants.

For some time, delta-8 THC products were a popular alternative for those looking for a mellower high, and a legal loophole to using THC products for recreational use as delta-8 is typically made by converting CBD into delta-8 through a chemical process.

With that being said, there are now more restrictions on delta-8 products, which means it’s no longer widely available. Additionally, the long-term effects of delta-8 THC use are not well-studied, and its use will still show up positive on drug testing for marijuana consumption.

TERPENE-BASED PRODUCTS

Terpene-based products, such as Sativa and Indica edibles, are derived from terpenes, aromatic compounds in various plants, including cannabis.

These products focus on harnessing different terpenes’ distinctive flavors and scents. Terpenes not only contribute to the aroma of cannabis but also have potential wellness properties.

Terpene-based products can include essential oils, tinctures, and concentrates emphasizing specific terpene profiles. Many terpene-based products will have labels that suggest their potential effects on relaxation, mood enhancement, focus, and more [57].

HEMP-DERIVED THC PRODUCTS

Thanks to the 2018 Farm Bill in the United States, hemp and its derivatives are federally legal as long as they contain no more than 0.3% THC by dry weight. This legislation makes for an interesting loophole in the creation of THC gummies derived from hemp that can produce mild psychoactive effects without exceeding the legal THC threshold.

THC gummies are crafted to harness the psychoactive potential of THC while sticking to the federal limit. This makes gummies a compliant and accessible option for those seeking the benefits of THC like pain relief. While the concentration of THC is low, these gummies can still provide noticeable effects for users.

Determining the “strongest” THC alternative is subjective and depends on personal preferences and tolerance levels.

For most people looking for the strongest THC alternative to support general endocannabinoid wellness without the psychoactive effects, and if our strongest THC edibles won’t work, then CBD may be the best option. Plenty of research surrounds this compound, and it’s more widely accessible online and in stores.

However, if one wants the psychoactive effects of THC, then one should look at hemp-derived THC gummies, which are legal by the 0.3% Farm Bill compliance, though they won’t be as potent as the traditional THC products made from marijuana. The legality of THC alternatives depends on the source it’s derived from and the laws in the local area of distribution and sale.

CBD, CBN, CBG, and certain THC products, when derived from hemp, are federally legal in the U.S. under the 2018 Farm Bill, as long as they contain less than 0.3% delta-9 THC by dried weight in the final product.

However, state laws may differ, with some states imposing restrictions or outright bans on certain cannabinoids, like delta-8 — regardless of whether it came from hemp.

It is always good to check the local regulations in the area where these products are sold to ensure compliance when considering the use of THC alternatives.

Just when breast is well established in transgender individuals MtoF, the natural remedies are abundant:

Feenus greek glyceric extract in guar gum gel (after evaporation of the aqueous phase of the gel the individual feels a good tension of the udder).

Dioscorrea villosa alcoholic tincture (it is more potent, but it is important not to spread this lotion near or upon the nipple, as the nipple absorption pramacodynamically is the most rapid of all the possible routes, after intravenous and conjunctival ones).

And, as aristocratic desmoiselles used in XVII century in France and Spain, for an ultra rapid lifting and brest up before a soirée de gala or a high society happening, the squeezing of the juice of unripe lime or tangerine on the nipples or the spreading of unripe tomato (green) upon all the udder.

A man (33 y old) who wanted to grow female and to boast about a beautiful and prosperous breast, followed an oral treatment of phytoestrogens derived from Agave sisaliana and legal cannabinoids and afterwards, in order to maintain his udders glamorous used to spread onto the breast unripe citrus spp. and tomatoes and other herbal remedies in the shape of cosmetic gélées or alcoholic lotions.
